In reference to the below email, for me to follow through on your complaint I would need a transcript of your phone conversations with Mr. Novak. From those phone conversations you will need to show me exactly what part you are basing your complaint on. Also, I would need to take a formal statement from you preferably in person so we can discuss in detail your complaint against Mr. Novak. Please respond with a date that you will have this completed. Once we have that information please send me a few dates that would work for you to meet.
Attached is the data request form in regards to number 2. Please complete and return that request form to me.
